Fracture rehabilitation
A large part of this caseload in Nakuru is post-fracture, and much of it traces back to the road. Highway collisions and boda boda accidents produce tibias, ankles, wrists, collarbones and the occasional very long recovery.
The pattern we see repeatedly: the fracture is treated well, the plaster comes off, and then nothing happens. The patient is told it will strengthen with use, which is only partly true. Six weeks of immobilisation produces stiffness and muscle wasting that needs deliberate, graded work to reverse.
Starting that late costs months. Starting it on time usually does not.
Working to a surgeon’s protocol

Before surgery is decided
For several common problems (degenerative meniscal tears, much shoulder impingement, knee osteoarthritis) structured rehabilitation performs comparably to surgery for a large proportion of patients. We will not tell you to avoid an operation you need, but a proper trial of rehabilitation is often worth attempting first, particularly when the surgery would mean travelling and paying for Nairobi.

Very. Weeks of immobilisation cause stiffness and marked muscle wasting, and it does not resolve on its own. This is exactly the point at which rehabilitation matters most and is most often skipped.
