The first six weeks decide a lot

Scar tissue matures, joints stiffen into whatever position they are held, and muscle around an operated limb wastes quickly. What happens in the first six weeks sets a ceiling that is difficult to raise later: knees especially, which stiffen readily and resist being freed afterwards.

The Nakuru problem specifically

A great many complex operations for Nakuru patients happen in Nairobi. People come home with a protocol, a follow-up appointment in six weeks, and no local rehabilitation arranged.

Those six weeks are the ones that mattered. By the follow-up the knee is stiff, the surgeon is disappointed, and everyone concludes the patient did not try hard enough. In our experience the patient tried perfectly hard and nobody told them what to do.

That gap is most of what this service exists to close.

The phases

  1. Protect and settle: swelling control, gentle range, safe walking
  2. Restore movement, full range before chasing strength
  3. Rebuild strength, progressive loading on both sides
  4. Return to function: stairs, driving, work, and sport where relevant

Skipping ahead is the commonest self-inflicted setback. So is stopping at phase three because the pain has gone.

Common questions

I had my operation in Nairobi and was discharged with exercises. Is that enough?

Usually not. A sheet of exercises with nobody checking your progress is the commonest reason people end up with a stiff joint. Rehabilitation needs someone reassessing and progressing it, and that can happen here rather than back in Nairobi.

When should it start?

Usually within days, and for joint replacements often before you leave hospital. Delay is the main cause of a poor final result.

Can you come to me while I cannot drive?

Yes, and that is exactly when it matters. The first weeks are when travel is hardest and consistency makes the biggest difference.
