Running Injuries at Altitude
Altitude does not injure you directly. It changes what a session costs and how fast you recover, and the mismatch is what shows up four weeks later as an Achilles.
What altitude actually does
At around 1,850 metres, the physiological cost of a run is higher and recovery between sessions is slower. That is the whole mechanism. Altitude does not damage tendons: it just means the same training plan leaves you more fatigued than it would at sea level.
Fatigued tissue tolerates load worse. Four to six weeks of that, and the Achilles or the calf that had been quietly grumbling becomes the reason you stop.
The first fortnight is the trap
The pattern is remarkably consistent in runners arriving from lower ground. They feel terrible in week one, conclude they have lost fitness, and add work to compensate. That is precisely backwards.
The first two weeks should be easier than normal, not harder. Effort-based rather than pace-based. Runners who accept a slower pace for the same effort adapt well; runners who chase their old numbers accumulate a deficit that shows up as injury.
What we see most
Achilles tendinopathy: the commonest by a distance, particularly in runners who also changed surface or shoes at the same time.
Calf strains, often on one side, and often in someone who has been running the same camber or the same direction repeatedly.
Patellofemoral knee pain, usually driven by weak hips rather than the knee itself.
Shin splints, mostly in people ramping mileage after a break.
Building up without breaking down
- Go by effort, not pace, for the first two to three weeks
- Make the easy days genuinely easy: this is the single biggest predictor
- Change one variable at a time: not new surface and new shoes and more mileage
- Add calf strength deliberately, straight-leg and bent-knee heel raises
- Treat persistent morning stiffness in the Achilles as a warning, not a niggle
When to get it looked at
Pain that persists beyond a week of easier running, pain that is worse first thing in the morning, or any pain that changes how you run. Achilles problems respond very well to loading when caught early and become genuinely stubborn when left for months: which is the difference between three weeks of managed training and a season.

Does altitude cause injuries?
Not directly. It raises the physiological cost of every session and slows recovery, so the same training plan produces more accumulated fatigue than it would lower down. The injury comes from the fatigue, not the altitude.
How long before I adapt?
Meaningful adaptation takes weeks, and the first ten days to two weeks are the highest-risk period: because that is when runners feel disproportionately bad, assume they are unfit, and train harder to compensate.
Should I run slower here?
At the same effort, yes, your pace will be slower, and that is correct rather than a problem. Runners who chase their sea-level paces in the first fortnight are the ones we see in the clinic a month later.
